Thank you for being a part of our Thanksgiving outreach! Before you fill out the form we wanted to bring your attention to a few things.

When you fill out the form below you will be emailed a list of supplies for the Thanksgiving bags. Everything on the list should cost roughly $15.00. Please allow a few minutes for the email to show up in your inbox. Be sure to check your spam folder.
DATES
AND
TIMES
The bags will need to be filled and returned by Sunday, November 17th. Turkeys will need to be dropped off on Monday morning, November 25, between 8-10 AM. Deliveries will be Monday afternoon on the 25th.
